Many times, discharge from hospitals or long term care facilities is only possible if a patient is able to arrange for homecare services. Families should ask questions to insure that a homecare agency will meet all their needs and be one in which they can place their trust.
Who can help me get the information I need so that my mother can come home?
Many families don’t know who to turn to when suddenly a family member is being discharged from an acute care or long term care facility. The social workers or discharge planners at the facility can be an excellent resource.
How do I know what kind of homecare agency to get for my mom?
You want to make sure that the agency you pick will meet all your mom’s needs because the need is not dictated by convenience, but by the patient’s inability to travel to an outpatient rehab center or clinic for daily medical interventions. If you can have all the services your mom needs provided by one organization, communication between all members of the therapy team is enhanced allowing for better coordination of care.
Aren’t they all the same?
No. You should try to find an agency that is licensed and accredited. Accreditation by the Joint Commission is an indication that an agency has gone through a rigorous process to assure the highest quality of care.
